Dear Valued Patient,

We hope this letter finds you and your loved ones safe and well.

We are pleased to share an important update about our primary care practice.

Effective May 1, 2026, Valley Medical Associates will become Baystate Primary Care and Pediatrics – Springfield as we join Baystate Health, a not-for-profit, integrated healthcare system serving more than 800,000 people across western New England.

The care you know, and trust will remain the same. You will continue seeing your current providers at our same location, 3640 Main Street, Suite 207, Springfield, Massachusetts, and your scheduled appointments, the practice phone number, and patient portal will not change.

What this transition does bring is expanded access and enhanced coordination of care. As part of Baystate Health’s integrated network of hospitals, specialists, and services, your primary care provider will be part of and connected to the region’s largest employed medical practice. This means improved communication among your care team, coordinated referrals, integrated support for chronic condition management, and a continued focus on preventive care and overall wellness. Being part of a larger, academically connected health system also allows us to draw on additional clinical expertise and resources while maintaining the personalized relationships that matter most.

We will continue to accept the same insurance plans we participate with today through April 30, 2026.

Beginning May 1, 2026, our practice will transition to Baystate Primary Care and Pediatrics – Springfield under Baystate Medical Practices. We encourage you to contact your insurance provider to confirm that your specific health plan allows you to continue receiving care at our practice.

You can do this by visiting your insurance company’s website or calling the Member Services number listed on the back of your insurance card. When you contact them, please be prepared to provide your provider’s full name and our updated group information:

Your medical record will remain securely available to your provider. If you prefer to transfer your care to another qualified provider in the community, or for any reason, you will need to complete and sign an Authorization of Release of Information form so that your records can be securely transferred to them.
